Pay: $2,560 to $2,720 per week Shift Information: Days - 5 days x 8
hours Contract Duration: 13 Weeks Start Date: ASAP About the
Position Pay Rate: $64.00 - $68.00/Hourly. Summary: We are looking
for a licensed Physical Therapist to join our team of health
professionals in providing excellent health care in our facility.
Typically, you'll be working with post-op patients to help them
with recovery. You will be consulting with them to learn about
their current condition and assess their patient evaluations in
order to develop proper treatment plans. Responsibilities:
?Performs evaluative and therapeutic procedures under the order of
a physician or other accepted healthcare provider (mandatory
supervision requirements) while observing precautions, indications,
and contraindications. Evaluates residents per department protocols
and establishes a therapeutic plan of care which includes
functional long- and short-term goals, frequency and duration of
services, and therapeutic modalities to be used. Seeks input from
the resident and primary caregivers regarding the therapeutic plan
of care. Incorporates this information into the therapy goals.
Provides consultation to the referring physician and other members
of the healthcare team regarding the plan of care and discharge
needs of the resident. Attends all pertinent team meetings.
Monitors and evaluates the treatment effect and progresses the
treatment program toward the functional goals established in the
treatment plan of care. Modifies the treatment plan of care as
necessary. Recommends and obtains appropriate assistive devices to
the resident ensuring that instruction has been given in their use.
Provide appropriate education to the resident and/or caregiver.
Documents the results of teaching. Requirements Requirements: 3 -
Year experience Required, Bachelor’s degree required. Masters or
Doctorate preferred. BLS Certification, Physical Therapy License.
